Source Program

Service Options Using Resources in Community Environments (SOURCE) is a program designed to provide elderly, disabled, and low-income persons receiving Medicaid SSI with a wide range of long-term health services, either in their homes or the community.
Program requirements are:

  • 65 years old or older and/or disabled

  • Receiving Medicaid and SSI

  • Medical conditions such as diabetes, high blood pressure, lung disease, arthritis, Alzheimer’s disease, dementia, heart disease, or stroke require monitoring and regular doctor’s care

Rhema Home Care, Inc. is licensed by Source to provide Personal Support Services. Affordable Medical Resources has been serving the community since 2003 and is currently serving 150 active clients. At Rhema Home Care, Inc. we work with our clients or their families to develop personally tailored services to best serve your needs. Our goal is to provide services that maintain or increase the functioning capacity of the clients being served and focus on the relationship between the clients and their needs.

Independent Care Waiver Program

Independent Care Waiver Program (ICWP)

Independent Care Waiver Program (ICWP) is a Medicaid waiver program, designed to provide services for individuals with physical and/or traumatic brain injuries. This program helps individuals stay in their community but provides them with the same level of care as being in a hospital.

Rhema Home Care, INC is licensed by ICWP to provide Personal Support Services. Because of our extensive experience in the field, we have the ability and resources to provide you with the specialized care required. Our team consists of well-qualified and experienced caregivers who are able to provide the specialized care needed by our clients and their families.

Community Care Services Program

Community Care Services Program (CCSP)

The Community Care Services Program (CCSP) is designed by Medicaid to provide Personal Support Services (PSS) to individuals needing assistance to carry out their Activities of Daily Living (ADL’s) due to an illness, injury or old age. These services provide members with the opportunity to stay in community and avoid nursing home placement. The intent of the service is to provide quality, competent care to CCSP members in the form of:

  • Personal Care

  • Housekeeping

  • Home management

  • Proper nutrition

  • Medically-related activities

  • Ambulation

  • Respite care to caregivers

Affordable Medical Resources, Inc. is a Private Home Care Provider licensed by Medicaid to provide Personal Support Services under the Community Care Services Program (CCSP). At Affordable Medical Resources, Inc. we work with our clients or their families to develop personally tailored services to best serve your needs. Our goal is to provide services that maintain or increase the functioning capacity of the clients being served and focus on the relationship between the clients and their needs.
